-SERVICE AGREEMENT-

My approach is respectful and aims to empower you to make the decisions that are right for you. I encourage you to ask questions and discuss any concerns you may have at the time of your session. This document is designed to inform you about the background, and theoretical approach of your psychotherapist, Natalie Roset, as well as to ensure that you understand our professional relationship. 

By entering into a counselling service provided by Natalie Roset, you acknowledge that you have read and understand the following:  

 

Background:

Natalie Roset is a PACFA registered Psychotherapist & Counsellor. Natalie has a BNursing and a GradCert in Palliative Care.

 

Records & Confidentiality:

All our communications may become part of the clinical record of treatment and will be appropriately secured. Your record is accessible to you upon request. All personal information gathered by Natalie during the provision of the psychotherapy service will not be disclosed except when: 

  1. it is subpoenaed by a court; or  

  2. failure to disclose the information would in the reasonable belief of Natalie Roset, place you or another person at serious risk to life, health, or safety; or  

  3. a) provide a written report to another professional or agency. e.g., a GP or a lawyer; or b) discuss the material with another person, e.g. a parent, employer, or health provider; or c) disclose the information in another way; or  

  4. disclosure is otherwise required by law.  

 

Professional Relationship:

Although our sessions may be very intimate emotionally and psychologically, it is important that the relationship is professional rather than a social to ensure safety through professional code of ethics.

 

Concerns:

If at any time and for any reason you are dissatisfied with the services then please contact Natalie. If a satisfactory resolution isn’t possible other alternatives will be provided.  

Emergency:

If you are having a medical emergency then call 000 or seek out services at your local hospital emergency room. If you are having a mental health emergency, please call Lifeline 13 11 14. For those experiencing an emergency outside Australia you can find help in your area by contacting www.befrienders.org. Natalie can accommodate individuals that are reasonably safe and resourceful. If at any time this does not feel like sufficient support, then we can discuss additional resources.  

Non-Emergency:

You may email natalie@natalie-roset.com. Natalie will respond when she is can.  Natalie endeavours to respond within 48hours.

Scheduling Appointments:

Appointments may be scheduled via the online bookings portal at www.natalie-roset.com

 

Cancellation of Appointments:

If you will not be able to keep an appointment you must notify Natalie at least 24 hours in advance of the scheduled session. If notice is not received, then you may be responsible for full payment of the session.

 

Fees:

My services will be provided to you at the rates listed on the booking portal of the website - www.natalie-roset.com. Medicare rebates are not applicable for Psychotherapists in Australia at this stage.

By entering into a counselling service provided by Natalie Roset, you acknowledge that if you have any concerns that might arise during the therapeutic process, you agree to raise them.  You also agree that whilst you are a client you will disclose any information about your ongoing safety.
